KAP Consent Forms

KAP providers have created a plethora of models for various consents for KAP treatment. Outside of official guidelines and protocols outlined for our practice, some practitioners have consulted lawyers exploring different priorities, concerns, and policies. The good news is that many practitioners are willing to share their documents with our growing professional community to help us crowd-source options for consent forms for your practice.

My forms tend to prioritize clear communication with clients. What that means is they make less use of legal language, and therefore may have less standing in court should that ever be required. Feel free to use or adapt based on your own needs, practice policies, and priorities.
